Apply for a Radio
To apply for a radio, landline speaker telephone, internet radio, or Amazon Echo, fill out the form to the right. You can email it to info@radioeye.org or mail to Radio Eye 1733 Russell Cave Rd. Lexington, KY 40505.
Your application must be approved by a physician, nurse, librarian, or other qualified person who knows the disability which qualifies you for our service. This can also be mailed or emailed separately from the qualified person.
